The creatures that inhabit Silent Hill range from spine-chilling to outright terrifying. While there are a handful of re-occurring monsters, many are exclusive to each entry in the series, and the personal nightmare of each main character. In fact, the developers of Silent Hill actually drew upon the human psyche when creating many of the monsters featured in the games, adding an eerie element of realism to their designs and attack patterns.

In Silent Hill 2, James Sunderland must face a myriad of monsters, some reflecting his own personal struggles, and others reflecting the struggles of other characters that he encounters. In addition to difficulty in defeating them, there is a clear hierarchy of scariness in which the demons of Silent Hill 2 fall under.

10 The Prisoners

Disembodied Whispers Haunt Toluca Prison Cells


The only thing scarier than what the player can see in Silent Hill 2 is what they can’t see. And this is only evident with prisoners in Silent Hill 2’s Toluca prison. In two of the cells, players will hear two voices chanting ritualistically, but looking in and around the cells, they will see nothing and nobody. The radio also won’t transmit static, as though the player might have encountered people, or their own imagination turning against them.

The player can still arm themselves and kill these ghosts, but they don’t actually present any tangible threat other than being creepy. Because of that, this does significantly lessen the threat of these particular monsters.

9 Creeper

The Ankle-Biters are Frightening, But Not Too Dangerous


If you’re entomophobic, Creepers are much higher in the ranking of scary monsters in Silent Hill 2. Otherwise, they are relatively tame in comparison to their cohorts. Creepers are large, cockroach-like creatures that emit a loud screech the closer they get to James.

Creepers are more annoying than scary and don’t attack in any grand type of way. Instead, they scurry forward and bite to attack. The worst part about Creepers is how difficult they are to hit, but once struck, they are defeated very quickly.

8 Mannequin

Mannequins are just that: monsters resembling mannequins that twist in grotesque contortions. While it is chilling to encounter the lurching figures in dark hallways, the overall design of mannequins is very straight-forward, even if they do go against what ordinary mannequins look like, as they lack a head and have another set of legs in place of arms.

Standing around James’ height, they aren’t very formidable, don’t use any substantial weapons, and attack in ways that are easy to dodge. The scariest thing about Mannequins is the awkward way that they move, especially when they are seen skittering along walls and ceilings, vanishing into the darkness as they hate the light. They were given a considerate buff in the remake, increasing their ability to dodge and the reach of their attacks.

7 Lying Figure

Standing or Crawling, the Lying Figure Remains Disturbing


The Lying Figure is the first enemy that James encounters in Silent Hill. It has a human-like figure (sans arms) with an open wound in its chest. Like the Mannequin, its design is very straightforward, but still disturbing in more ways than one. Lying Figures limp towards James in disturbing ways to attempt to lunge at him, and also have the ability to shoot bile at him from their open chests.

However, the scariest part about Lying Figures is their ability to scurry across the ground after they are struck. When they are downed, they may suddenly switch from walking to crawling at a disturbingly quick pace, emitting a loud screech each time they change direction. This makes them not only difficult to strike, but disturbing to watch. In addition, certain Lying Figures expire in a burst of acid in the remake’s otherworld, dealing damage even when dying.


6 Bubble Head Nurse

As Iconic As It Is Frightening

Bubble Head Nurses are the twisted incarnations of the nurses of Brookhaven Hospital and are one of the more well-known reoccurring monsters in the series. In Silent Hill 2, they are armed with metal pipes and produce shrieks and gurgling noises as they approach James with their iconic yet terrifying shambling steps.

Their bloody, decaying appearance, the noises that they make, and the fact that they often appear in groups are all factors that make them one of the more frightening monsters to encounter in the game. They prove to be one of the most durable mob enemies, requiring a few more hits to take them down, and sometimes even standing back up just when the player thinks they have finally killed them.


5 Flesh Lips

The Player Will Be Trapped in the Room with this Monstrosity

James encounters Flesh Lips in Brookhaven Hospital after Laura locks him in a room with them. When the Flesh Lips are encountered, they are a boss battle and James must defeat them in order to escape the room. Hanging from the ceiling, Flesh Lips resemble large piles of human flesh in what appears to be a hospital bed, with dangling feet and lips at the bottom that it uses to attack with.

Its disturbing features, coupled with its presence swinging from the ceiling, make it one of the more chilling monsters that appear in the game. And being locked inside the room with them only makes it all the more frightening, as there is no escape from these monstrosities.


4 Mandarin

Even Beneath Floors, they are Terrifying

The design of the Mandarins is genuinely disturbing. With two large arms that have lips, they reach from underground and attack James from beneath him. Their placement makes them difficult to strike, and the dark emptiness beneath them leads players to wonder about their unsettling origin.

The fact that Mandarins are displaced from James yet still pursue him gives them an eerie presence that not many other enemies in the game have. The idea of a creature reaching up from beneath to attack the player is not only genuinely startling, but also provides one challenging fight that players will want to avoid. They were given more appearances in the remake, which did not stop them from being so aggressively scary whenever they were on-screen, especially when they are stripped of their weapons in the hotel.


3 Mary

The Final Boss is a Nightmarish One

Despite being the final boss of Silent Hill 2, Mary is not the scariest monster in the game, though she is still frightening to face. When James confronts Maria at the end of the game, she is still in her human form; however, she quickly transforms into a grotesque, corpse-like monster suspended in the air.

The overall design of Mary centers around the theme of her illness, as she is bound to a hospital bed frame that has spikes protruding from it. The remake adds a few extra sharp spikes that are not too dissimilar to insect legs, which make her even more frightening to face.


2 Abstract Daddy

This Boss Fight Symbolizes Something Truly Horrific

The most disturbing boss in Silent Hill 2, and perhaps one of the most disturbing in the entire franchise, is the Abstract Daddy. It is the physical incarnation of Angela Orosco’s trauma. Its appearance is also likely to cause some trauma, as it appears like two figures that have melded together and into a bed, creating a terrifying entity.

The Abstract Daddy is faced in a very tight space, is difficult to defeat, and attacks in disturbing ways. If it gets close enough to James, it lunges at James and confines his head in its body. Its boss fight was slightly changed for the remake, pursuing James through a series of tight corridors before the main portion of the boss fight.


1 Pyramid Head

The Iconic Monster Who Haunts the Franchise

Pyramid Head is arguably the most famous character in the entire Silent Hill franchise; it even appears as a DLC character in the asymmetrical horror game Dead By Daylight. Donning its iconic metallic, triangular helmet, Pyramid Head primarily attacks its prey with a giant sword that it drags across the floor.

Pyramid Head is not a singular entity, but multiple incarnations of the guilt and grief that James feels due to his relationship with Mary. The penultimate boss of Silent Hill 2 pits James against two Pyramid Heads, providing both a very terrifying, difficult and symbolic fight. Whenever Pyramid Head is on-screen, true terror ensues. The Silent Hill 2 remake only made Pyramid Head more oppressive. He has more appearances, acting like a stalker enemy to the dismay of players who have a heart attack whenever they turn a corner to see it lurching forward.
